Issue: Multifactor OTP Email check succeeded not shown
Description: A user was unable to log in using email OTP for MFA.
As a workaround, we deleted the user’s OTP entry from the MFA table so they could set it up again. This is the same workaround we have used for other users who had trouble logging in with email OTP.
After that, the user appears to have re-added email OTP successfully, based on the latest changes in the user history. However:
no success check message was shown after setup no error was recorded in the instance Event table for that user
Expected Behavior: After the user re-adds email OTP successfully, the system should display a success confirmation/check message and log any relevant success or failure events properly.
Actual Behavior: The user appears to have completed OTP setup, but there was no success confirmation shown and no related error event logged.
Steps to Reproduce:
User fails to log in with email OTP. Delete the user’s OTP entry from the MFA table. User sets up email OTP again. Check user history for the latest changes. Observe that no success check message is shown. Check the instance Event table and observe that no error is logged for that user.
